Before Your Visit

Can I get my lab tests done through my primary care doctor?

No. Effective September 2025, all the lab tests for immigration medical must to be ordered by the civil surgeon as per CDC and USCIS. Not doing so will result in rejection of I-693 application!

What if I got all the required vaccines in the past but I don't have records?

For Polio and Tetanus(Tdap) vaccines, no immunity tests are acceptable. Hence, if you don't have records for these, you will have to get one dose of these vaccines for your I693 completion.

For the MMR, chickenpox(Varicella) and Hepatitis B; immunity tests done within last 1 year are acceptable alternatives. We offer order slip for the immunity tests at LabCorp at the best possible price:
  • MMR immunity test - Needed for everyone born in or after 1957 who doesn't have proof of vaccination
  • Varicella immunity test - Needed for everyone who doesn't have proof of vaccination
  • Hepatitis B immunity test - Needed for everyone under 60 years of age who doesn't have proof of vaccination

If you're not immune to MMR/ Hepatitis B/ Varicella on the lab tests, then you will need to get a vaccination for it.

Do I need to have new 2024-2025 Covid-19 vaccine for I693?

Effective 1/22/2025 Covid-19 vaccine is not needed for I693.

How many doses of the vaccine are required to complete I-693?

One dose is enough as long as it is not time for the second dose prior to your appointment. Second dose of Polio vaccine, Hepatitis B vaccine and Varicella vaccine is due 4 weeks after the first dose and hence, you will need a second dose of these vaccines if more than 4 weeks have been elapsed at the time of your appointment with the doctor since your first dose.

We have appointment availability every week, depending on location, but you should complete your labs and vaccine prior to coming to your appointment.

You must not delay the appointments more than 2 weeks after labs have been collected. We will accommodate you to one of our locations accordingly, but you must be willing to travel and take the appointments once offered. No location or time can be guaranteed. You must be flexible to accept the appointment as per above timeline.

This way you will not need to come to our office multiple times and you can complete your I693 in one go in the fastest time possible.

What do I need to bring to my appointment?

Bring one valid government issued photo ID such as driver's license, state ID or passport.

The Exam & Your Form

How long does the exam take and when do I receive my sealed I-693 form?

If all records are available at the time of appointment and no further work-up is needed, we can give you the signed I-693 form sealed in an envelope SAME DAY in about 20 minutes!

Can my sealed envelope be mailed to me or someone else pick it for me?

No! All applicants must come in person to sign and get their own sealed envelope as required by USCIS!

Do you perform the TB test or blood test?

Effective 9/5/2025, all labs must be ordered from Civil Surgeon office within 2 weeks of your appointment for completion of I693.

  • Quantiferon TB test - Needed for everyone of age 2 years or above
  • Syphilis test - Needed for everyone of age 18 years to 44 years
  • Gonorrhea test - Needed for everyone of age 18 years to 24 years

After completing your Requirements Preparation Package purchase, you will receive instructions by email. Any of the above laboratory testing which is required to be issued from our office, we will send you a lab order for you to complete testing at an independent CLIA-certified laboratory (Quest Diagnostics or LabCorp).

The laboratory will collect your sample and send results directly to our office. We will notify you once your results are reviewed.

Is a chest x-ray required?

No, not unless you have a positive tuberculosis lab test result.

Special Situations

What if I am pregnant?
If you are pregnant, you are not required to take following vaccines for I693:
  • MMR
  • Varicella
  • Polio

Hepatitis B vaccine and Tetanus vaccine can be safely taken during pregnancy.
All the above vaccines can be taken during breastfeeding.

I don't have original vaccination records but can I get a certificate from another doctor stating that I have completed all childhood vaccination?

A statement or certificate from another doctor regarding your childhood vaccinations, in the absence of the original vaccination records, may be considered fraudulent by USCIS. We kindly request that you provide the original vaccination records. If you do not have them, you may either receive an additional dose of the vaccine or provide proof of immunity by lab tests (lab tests for immunity are applicable only for MMR, Hepatitis B and Varicella).

Do you accept medical insurance or travel insurance for the immigration physical exam?

No. Immigration medical exams are not covered under any insurance plans. We do not deal with any insurance but you can submit the receipts for the fee paid to us your insurance to see if they will cover or not. We do not communicate with any insurance for anybody. You can go to any pharmacy to get the vaccines using your insurance. We don't sell vaccines.

What if I or my family members do not have medical insurance?

If you do not have insurance, you may go to your local pharmacy or health department (Find a health center here.) to get your vaccinations. You can also order your lab tests yourself online, the links for required tests are provided here.
