What Is Form I-693?

Form I-693, Report of Immigration Medical Examination and Vaccination Record, is used by USCIS to document the results of the immigration medical examination required for certain immigration benefits, including adjustment of status.

The examination evaluates whether an applicant has a health-related condition that may affect admissibility under U.S. immigration law.

Only a physician currently designated by USCIS as a Civil Surgeon may perform the immigration medical examination and complete Form I-693 in the United States.

What Laboratory Tests Are Required for I-693?

Laboratory requirements depend primarily on your age and medical history.

For most applicants:
